Position Overview: You will provide direct psychosocial and practical support to patients and families, helping them navigate complex medical, emotional, and social challenges.
You’ll collaborate closely with patients, families, physicians, case managers, and other healthcare professionals to deliver holistic care and improve patient outcomes in a fast-paced academic healthcare environment.

Location: Johns Hopkins Kimmel Cancer Center located in Baltimore, MD

Schedule: Monday - Friday 8:00 AM - 4:30 PM

• Conduct comprehensive biopsychosocial assessments
• Identify patient needs and develop individualized care plans and interventions
• Provide crisis counseling, suicide assessment, and short-term counseling
• Support care coordination and address social determinants of health
• Assist with complex discharge planning and mitigate psychosocial barriers
• Facilitate patient and family education and engagement
• Lead and participate in family meetings and advance care planning discussions
• Provide support around palliative care, end-of-life planning, grief, and loss
• Collaborate with interdisciplinary teams to optimize patient care

• MSW Social Work from an accredited school of social work.
• Must be licensed by the Maryland Board of Social Work Examiners at the LMSW or LCSW-C level.
• Must have Epic Experience 
• Minimum of 2 years of experience in providing clinical social work service in a Oncology, pallative care, or Hospice Hospital setting is required.
• Experience with hospital discharge planning is a requirement.
