Position Overview:The Clinical Dietitian is a key member of the interdisciplinary care team at Hopkins ElderPlus (PACE), providing individualized, evidence-based nutrition care to older adults. This role supports health, independence, and quality of life through assessment, care planning, and medical nutrition therapy for participants with complex and chronic conditions across the PACE center, home, and community settings.

Schedule:

  • Monday- Friday 8:00 am - 4:30 pm

Responsibilities

 
  • Conduct comprehensive nutrition assessments and periodic reassessments for PACE participants in accordance with program standards and regulatory requirements
  • Develop, implement, and evaluate individualized nutrition care plans that address participants’ medical conditions, functional status, cultural preferences, and psychosocial needs
  • Provide evidence-based medical nutrition therapy for acute and chronic conditions, including, but not limited to, diabetes, cardiovascular disease, renal disease, malnutrition, dysphagia, and gastrointestinal disorders
  • Collaborate as an active member of the interdisciplinary team to support coordinated, participant-centered care planning and service delivery
  • Monitor nutritional status through ongoing review of weight trends, laboratory data, clinical indicators, and treatment outcomes; modify care plans as clinically indicated
  • Provide nutrition education and counseling to participants, caregivers, and families to promote understanding, adherence, and self-management
  • Support menu planning, therapeutic diet implementation, and food service operations to ensure nutritional adequacy, quality, and compliance with CMS, PACE, and organizational standards
  • Maintain accurate, timely documentation of assessments, interventions, and outcomes in the medical record in compliance with PACE regulations, CMS guidelines, and Johns Hopkins policies

 

Qualifications

  • Bachelors Degree from a accredited college.
  • Registered Dietitian (RD) by the Comission on Dietetic Registration 
  • Licensed Dietitian-Nutritionist (LDN) in Maryland 
  • CPR Certified
  • Two years of Registered Dietician experience and one year of relevant experience to the PACE program are required 
  • Reliable transportation due to possible Home visits.
