Position Overview: Performs comprehensive psychiatric assessment and treatment of child or adult outpatients, 18 years of age and older, who have a wide variety of psychiatric illnesses so as to alleviate emotional distress and enable those patients to live more satisfactorily in the community.
Location: Johns Hopkins Hospital located in Baltimore, MD
Schedule: 20 hours a week.Weekday shifts are 5-11:30pm and weekend is 3-11:30pm. Every other weekend required.

Provide social work support to Adult Inpatient/ Outpatient Rehab Medicine.
Works in collaboration with additional Social Work and Nurse Case Management partners.
Responsibilities include: early identification, high risk screening and readmission screening of all patients.
Services include, but are not limited to adjustment to: illness counseling, safety assessments, discharge planning, identification of obstacles and barriers to discharge through proactive screening and assessments, and coordination of care with multidisciplinary team.
Shared responsibilities for Care Management coverage during absences for all inpatient units within the Department of Care Management which falls under the Dept of Care Coordination and Clinical Resource Management and is approximately 700 beds.
