We are currently seeking an experienced Social Worker for a temporary position at Johns Hopkins Bayview Medical Center on an Adult, Inpatient Med/Surg unit.
Provides direct psychological, social and practical care to a select caseload of patients and families including social work assessments, problem identification, goal setting and implementation of care plans. Makes referrals to outside agencies and coordinates continuity of care. Works collaboratively with and provides leadership with the multidisciplinary team to optimize patient outcomes. Serves as a clinical resource expert in a specialty area.
Will work on Inpatient Adult Medicine unit. Will work closely with RN case manager.
Psychosocial assessment of new patients. Counseling as required.
Referral for and assistance with securing medications, transportation, home services, referrals for home care and home hospice.
Family meetings for support and goals of care.
Provides direct service to patients including assessments, family meetings, end of life care.
Provides discharge planning for patients going to skilled vent rehab, home and hospice care.
MS Social Work from an accredited school of social work.
Must be licensed by the Maryland Board of Social Work Examiners. LCSW-C preferred; will consider LGSW.
2 years or more of licensed clinical social work experienced in an acute hospital setting. Previous experience working on an inpatient, adult medical service. Critical care experience preferred.
