Overview

Provides direct psychological, social and practical care to a select caseload of patients and families including social work assessments, problem identification, goal setting and implementation of care plans. Makes referrals to outside agencies and coordinates continuity of care. Works collaboratively with and provides leadership with the multidisciplinary team to optimize patient outcomes.  Serves as a clinical resource expert in a specialty area.

Full time, 8-hour days.    8:30 -5:00.  

 

 

 

Responsibilities

The successful candidate will work on the adult inpatient psychiatric unit at Johns Hopkins Bayview Medical Center.  

Primary responsibilities will include completion of psychosocial assessments, involvement of patients in the development of individualized discharge plans collaboration with the multidisciplinary team, and referrals to identified treatment and support services in the behavioral health system.  

Qualifications

MS Social Work from an accredited school of social work.

 

Must be licensed by the Maryland Board of Social Work Examiners.  LGSW or LCSW-C.

 

Previous experience working on an inpatient, adult medical service.   Critical care experience preferred.  

 

Strong written and oral communication skills are necessary for timely referrals and documentation in the medical record.  

 

Must have working knowledge of psychiatric disorders as they related to treatment and discharge planning.
